What Vitamin D Does and Why It Matters

Vitamin D behaves more like a hormone than a vitamin. Your skin makes it from cholesterol when UVB rays from sunlight reach it, and it acts on receptors found in almost every tissue. There are two forms: D2 (ergocalciferol, from plants and some fortified foods) and D3 (cholecalciferol, from sun, animal foods and most supplements). D3 is the more potent and preferred supplement form.

Its main job is managing calcium. Vitamin D is converted in the liver to 25-hydroxyvitamin D (the form measured in your blood test), then in the kidney to its active form, calcitriol. Calcitriol tells your gut to absorb calcium and phosphate, raising absorption from around 10 to 15 per cent up to 30 to 40 per cent. Without enough vitamin D, your body cannot use dietary calcium properly even if you eat plenty, which is why low vitamin D quietly undermines bone health.

The clearest consequences are skeletal. In children, severe deficiency causes rickets (soft, bowed bones and delayed growth), now rare but still seen in low-income groups. In adults it causes osteomalacia (soft, aching bones and muscle weakness) and adds substantially to osteoporosis after menopause. For Indian women, low calcium intake plus low vitamin D is a double hit on lifelong bone strength.

Beyond bone, vitamin D receptors sit in immune cells, muscle, heart, brain and gut. Skin pigmentation. Melanin acts as a natural sunscreen, so darker skin needs 3 to 6 times longer in the sun to make the same vitamin D as fair skin. The 10 to 15 minutes often quoted as enough is based on fair-skinned populations; for the typical wheatish-to-brown Indian skin tone, 30 to 60 minutes or more is needed.

Indoor lives. Most urban Indian women spend their waking hours indoors, with office work, study, housework and screen time. The little outdoor time they get is often the early morning or evening, exactly when UVB is weak. Midday sun (10 am to 3 pm), when UVB is strongest, is when most people are inside.

Sun avoidance for skin tone. A cultural preference for fair skin drives active sun avoidance, generous sunscreen use, umbrellas, scarves and full sleeves. SPF 30 or higher blocks 95 per cent or more of UVB and effectively stops vitamin D synthesis. How Much Sun You Actually Need

Sun is free vitamin D, but how much you need depends heavily on your skin tone, the season and where you live, and it has to be balanced against skin protection.

Time by skin tone (midday sun, around 25 per cent of the body exposed: face, arms and lower legs). Fair skin that burns easily needs about 10 to 15 minutes; light brown skin about 15 to 25 minutes; medium brown (typical North and Central Indian) about 20 to 40 minutes; brown to dark brown (typical South Indian) about 30 to 60 minutes; very dark skin about 45 to 90 minutes. Use these as rough guides, not prescriptions.

Timing matters more than people expect. Only midday sun (10 am to 3 pm) carries enough UVB to make vitamin D. Early morning and evening sun gives UVA but almost no UVB, so the gentle morning sun that feels kind to the skin is actually the least useful for vitamin D. The hotter midday sun most people avoid is the effective one.

Frequency. Two to three sessions a week is enough; daily exposure is not needed, because the body stores some vitamin D between sessions. The more skin exposed, the more you make per session, so a private terrace or garden where you can expose arms and legs is more efficient than a fully covered walk outdoors.

Supplement Protocols: Daily, Weekly, Injection

For most Indian women, sun is not enough and a supplement is needed. Three well-established protocols cover almost every situation. Always take the form that matches your goal: maintenance, correction, or rapid correction.

Daily maintenance. 1,000 to 2,000 IU of D3 a day, taken with a meal that contains some fat for better absorption. This keeps levels steady once you are sufficient and is the right long-term dose for prevention. Children need less (400 to 600 IU); some older adults need more (2,000 to 4,000 IU). Typical cost is roughly 100 to 500 rupees a month.

Weekly correction for diagnosed deficiency. 60,000 IU once a week for 6 to 8 weeks, then switch to the daily maintenance dose. This is the standard Indian approach when a test shows 25(OH)D below 30 ng/mL. The sachet is mixed in water or milk and taken on the same day each week (Sunday is a common choice). Mushrooms are the only meaningful plant source, and only when UV-treated: sun-exposed or UV-treated mushrooms can reach up to 400 IU per 100 g, while ordinary cultivated ones have very little. Laying fresh mushrooms gills-up in direct sun for 30 to 60 minutes before cooking genuinely boosts their content.

Fortified foods add small amounts: fortified milk (such as some Amul and Mother Dairy products) provides about 40 to 80 IU per cup, and some fortified atta and breakfast cereals add 40 to 200 IU per serving. Fortification in India is voluntary and patchy, so read labels.

Realistically, a non-vegetarian who eats fish, eggs and fortified foods might get 200 to 400 IU a day from food; a vegetarian 50 to 150 IU; a vegan 50 to 100 IU. All fall short of the 600 to 1,000 IU target, which is why supplementation remains the dependable part of the plan, much as it is for calcium across life stages.

Testing Your Levels and Reading the Results

Vitamin D status is checked with the 25-hydroxyvitamin D (25(OH)D) blood test, which reflects your total stores and is the standard clinical measure.

Reference ranges. Sufficient is above 30 ng/mL (some endocrinologists aim for 40 to 60 ng/mL); insufficient is 20 to 30; deficient is 10 to 20; severely deficient is below 10. Levels above 100 ng/mL are excessive and need evaluation. These cut-offs are used by the Indian endocrine community and major international bodies.

Requirements. The ICMR-NIN recommendation in pregnancy is 600 IU a day (some bodies advise 1,000 IU). Because most Indian pregnant women start out deficient, the usual protocol is a 25(OH)D test at booking, then either a 600,000 IU injection (free at PHCs) or 60,000 IU orally each week for 6 to 8 weeks if deficient, followed by 1,000 to 2,000 IU daily for the rest of pregnancy. Most prenatal multivitamins already include 400 to 1,000 IU to support that maintenance.

Benefits. For the mother, adequate vitamin D supports the higher pregnancy calcium need and is linked with lower risk of pre-eclampsia and possibly gestational diabetes, and fewer aches. For the baby, it supports bone, lung and immune development and is associated with lower risk of low birth weight.

The newborn link. A vitamin D-deficient mother tends to have a vitamin D-deficient baby. Breast milk is naturally low in vitamin D, infants should not have prolonged sun exposure, so the Indian Academy of Pediatrics recommends 400 IU of vitamin D daily for all infants from birth, breastfed or formula-fed, until dietary intake is adequate.

Breastfeeding. A nursing mother still needs 600 to 1,000 IU a day; the standard approach is the mother taking 1,000 to 2,000 IU while the infant takes 400 IU separately. Within recommended doses, vitamin D and the Arachitol injection are both safe in pregnancy and breastfeeding, and vitamin D and calcium can safely be taken together. Signs of Deficiency and When to See a Doctor

Vitamin D deficiency often hides behind vague symptoms that are blamed on a busy life or ageing, then improves noticeably within weeks of correction, which can make the link clear in hindsight.

The most recognisable signs are musculoskeletal: a dull, aching bone pain (often low back, hips or ribs, worse at night and in cold weather); muscle weakness in the hips and thighs that makes stairs or rising from a chair harder; cramps; and tiredness out of proportion to activity. Many women also describe persistent fatigue, poor concentration or 'brain fog', and low mood that can worsen in winter.

Other associations include more frequent respiratory infections, hair shedding (telogen effluvium), and a possible link with menstrual and fertility problems including PCOS, although the evidence here is still emerging. Vitamin D is rarely the sole cause of hair loss or low mood, so other causes deserve a look too.

Long term, untreated deficiency raises the risk of osteomalacia and osteoporosis, and of fractures and falls in older age, with the heaviest impact when combined with low calcium intake.

Vitamin D is among the most affordable health fixes in India, and access keeps improving across both government and private channels.

Free government options. Antenatal programmes provide the 600,000 IU injection free at PHCs for pregnant women with documented deficiency.
